Whenever a new patient comes in for fertility support, one of the first questions they ask is: “How often do I need to come, and for how long?”
Acupuncture works best when it’s done consistently, and aligned with both your cycle and the path you’re on — whether you’re trying naturally or in the middle of an IVF cycle. After 25 years of working alongside New York City’s fertility clinics, we’ve developed timelines that match where you are. Here’s how we think about it.
Why 90 Days Matters
Did you know that the eggs that ovulate this month started their final maturation phase roughly 90 days prior? Sperm follow a similar 72-to-90-day cycle. This window — the three months before conception — is when blood flow, hormones, nutrient delivery, and oxidative stress most influence the quality of the eggs and sperm that will eventually meet. It’s also why we recommend starting acupuncture at least three months before you plan to conceive or begin an IVF cycle. The earlier you start, the more groundwork we can lay.
The Pre-Conception Phase: Three to Four Months Out
This is the foundation phase, and it’s the same regardless of whether you’re planning to conceive naturally or with assistance. We typically recommend weekly acupuncture treatments for at least 12 weeks before you start actively trying. During this phase, we’re working to improve ovarian blood flow, regulate hormones, calm the nervous system, reduce inflammation, and identify any underlying Chinese medicine patterns that need attention. Many of our patients also use this time to begin a customized Chinese herbal formula, which extends and deepens the effects of acupuncture between sessions.
Trying Naturally
Months 1–3: weekly acupuncture, plus Chinese herbs if prescribed.
Once you’re actively trying: weekly acupuncture, with timing oriented around your cycle. We pay particular attention to the follicular phase (cycle days 1–14), the ovulatory window, and the luteal phase (cycle days 15 onward), adjusting points and treatment focus accordingly.
Once you conceive: we shift into early pregnancy support, typically every one to two weeks for the first trimester.
Most natural conception patients we work with do best with weekly sessions throughout. Spacing further apart than every two weeks tends to dilute the cumulative effect of treatment.
IUI Schedule
Three months before IUI: weekly acupuncture during the pre-conception phase.
Cycle of the IUI: one to two sessions in the days leading up to insemination, ideally including the day of the IUI or the day after, to support uterine receptivity.
Post-IUI: weekly sessions through the two-week wait, then transitioning into pregnancy support if conception occurs.
IVF Schedule
IVF is the most studied application of fertility acupuncture, and the schedule is more intensive — but the research strongly supports it. One study found that women who attended an average of 12 acupuncture sessions during their IVF cycle had 73 percent higher odds of a live birth. Here’s how we typically structure it:
Three months before IVF: weekly acupuncture as part of the pre-conception phase, focused on egg quality, blood flow, and stress reduction.
During suppression: weekly sessions to support hormonal balance and reduce side effects.
During stimulation: twice-weekly sessions to support follicular development and ovarian blood flow.
Egg retrieval: a session within a day or two of retrieval to support recovery.
Transfer window: a session before transfer and another within 24–48 hours after transfer — this is the protocol with the strongest research support.
Two-week wait and beyond: weekly sessions through the wait, transitioning into pregnancy support if you conceive.
Tailoring the Schedule to You
These are starting points, not formulas. We adjust based on your age, your diagnosis, your treatment timeline, and how your body responds. Patients with PMOS (formerly PCOS) often benefit from longer pre-conception phases. Patients with diminished ovarian reserve sometimes need more intensive support during the stimulation phase. Patients with a history of pregnancy loss may continue acupuncture more frequently through the first trimester. Your practitioner will outline a plan after your initial consultation.
What the Research Shows
Beyond the IVF live-birth data, studies have shown that patients receiving more than three acupuncture sessions during their IVF cycle have 50 to 60 percent higher pregnancy rates than those receiving fewer treatments. Earlier work from the University of Gothenburg and a well-known German study found pregnancy rates of 42 percent in IVF patients receiving acupuncture, compared with 26 percent in those who did not. The research is clearest for IVF, but the underlying mechanisms — better blood flow, hormonal regulation, lower stress — apply just as much to natural conception and IUI.
Frequently Asked Questions
How early should I start fertility acupuncture?
Three to four months before you plan to conceive or begin an IVF cycle is ideal. If you’re starting later than that, don’t worry — there’s still meaningful benefit, especially during the stimulation and transfer windows.
What if I’m mid-cycle when I start?
That’s fine. We meet you where you are. Even mid-cycle acupuncture supports stress reduction, blood flow, and hormonal balance. We’ll build a plan from your current point forward.
How long are sessions?
Initial Consultations are 90 minutes to allow for a thorough intake. Follow-up treatments are typically 45 to 60 minutes, with the needles in place for about 25 minutes while you rest.
I can’t come weekly. What’s the minimum that still helps?
Every two weeks is the lower limit at which we tend to see meaningful progress. Less frequent than that, the cumulative effect drops off. If a weekly schedule isn’t possible, we’d rather concentrate sessions around key windows (stimulation, transfer, ovulation) than space them out arbitrarily.
